VA Office of Inspector General issues report, recommendations for Omaha facility

OMAHA, Neb. (WOWT) - The Department of Veterans Affairs’ Office of Inspector General (OIG) issued a report on an Omaha facility on Thursday.

In a report submitted Thursday, the OIG says they conducted an inspection of the VA Nebraska-Western Iowa Health Care facility in Omaha.

The inspection came at the request of Nebraska Congressman Don Bacon to evaluate allegations concerning the facility’s inpatient mental health unit.

Among their recommendations, the OIG says the facility should:

  • Install night-lighting changes
  • Implement guidance concerning the use of handheld flashlights
  • Implement patient safety observation rounds consistent with VHA guidelines
  • Develop mitigation plans as lined out by the Mental Health Environment of Care Checklist
