The Nebraska Public Service Commission plans to look into the phone and 911 service outage last week that affected thousands of people across southeast Nebraska.
On Tuesday, a Windstream Communications official discussed with the commission the outage that interrupted service in 12 counties for about 15 hours Thursday.
Windstream vice president Brad Hedrick said a necessary upgrade to an internal timing system caused the failure.
Asked whether other companies could experience similar failures, Hedrick said all companies use essentially the same system, so the potential exists.
Commission Chairman Gerald Vap says he intends to order a study on the possibility.
The commission expects a report from Windstream on the incident within 30 days.
