An Omaha day care provider has had her license taken away. Authorities allege unsanitary conditions and they say that a resident of the house has been charged with a felony.

The daycare is located at 20th and Martha and was run by Andra Callis.

The Douglas County Health Department claims to have found cleaning agents and poisons were not locked and kept separated from food. There were rodent droppings on the kitchen floor, counters, carts and in the dishes. There were allegedly accident hazards such as broken glass on the window pane, boards with exposed nails, and lawn equipment.

In addition, the provider's brother, Louis Callis, was arrested and charged with felony abuse to a vulnerable adult. He lives in the home.

Andra Callis will not be eligible to reapply for a child care license for the next two years.
