I don't know about your place but this winter just hammered my driveway.  I have little divots all over the place on the panels closest to the street.  It looks like it had a case of bad acne.

The concrete is less than four years old.  I suspect the freeze and thaw cycle this year pushed it to the limit.

I never use ice melt so I know that's not the cause.  Neighbors are seeing similar problems -- at least the ones with south facing driveways.

As bad as potholes have been this year, I guess it's only appropriate to see this closer to home.
